Assessment responses and clinician-reviewed results constitute protected health information (PHI). We handle PHI in accordance with HIPAA requirements. PHI is stored using industry-standard encryption and is only accessible to authorized clinical personnel.
3. How We Use Your Information
We use your information to: (a) provide and improve our assessment and monitoring services, (b) facilitate clinician review of your assessment, (c) communicate with you about your care, (d) comply with legal obligations, and (e) generate de-identified aggregate data for research purposes.
4. De-Identified Research Data
We may use de-identified, aggregated data for research and platform improvement. This data cannot be traced back to any individual. De-identified research rights are separate from identifiable testimonial or media rights, which require explicit, separate consent.
5. Data Sharing
We do not sell your personal information. We share information only with: (a) licensed clinicians involved in your care, (b) service providers necessary for Platform operation (hosting, email), and (c) as required by law or to protect safety.
6. Data Security
We implement administrative, technical, and physical safeguards to protect your information. All data is encrypted in transit and at rest. Access to PHI is logged and audited.
7. Your Rights
You have the right to: (a) access your personal data, (b) request corrections, (c) request deletion (subject to legal retention requirements), (d) revoke consent for non-essential data processing, and (e) receive a copy of your data in a portable format.
8. Cookies and Tracking
We use essential cookies for Platform functionality and analytics cookies to understand usage patterns. You can control cookie preferences through your browser settings.
9. SMS/Text Message Data
We collect your phone number when you opt in to receive text messages. Phone numbers are used solely for sending assessment, monitoring, and care-navigation messages. We do not share phone numbers with third parties for marketing purposes. Phone numbers are not shared with or sold to third parties. SMS consent is not a condition of purchase or use of the Platform. You can revoke SMS consent at any time by replying STOP to any message or by contacting us at support@theresetregimen.com.
