AS A PATIENT OF THE RESET REGIMEN, YOU HAVE THE FOLLOWING RIGHTS. THESE RIGHTS APPLY TO ALL SERVICES PROVIDED THROUGH OUR PLATFORM, INCLUDING ASSESSMENT, CLINICIAN REVIEW, AND ONGOING MONITORING.
Your Rights
- Right to Respectful Care: You have the right to considerate and respectful care, free from discrimination based on race, ethnicity, gender, sexual orientation, age, disability, religion, or socioeconomic status.
- Right to Information: You have the right to obtain complete and current information about your assessment results, care routing, and any recommended protocol in terms you can understand.
- Right to Know Your Clinician: You have the right to know the name, credentials, and qualifications of the clinician reviewing your assessment.
- Right to Privacy: You have the right to privacy in all aspects of your care. Your assessment responses, clinical data, and routing decisions are protected health information.
- Right to Consent: You have the right to informed consent before any clinical services are provided, including a clear explanation of what the service involves and its risks and benefits.
- Right to Refuse: You have the right to refuse any recommended treatment or service. This refusal will be documented but will not affect your right to future care.
- Right to Your Records: You have the right to access, review, and obtain copies of your medical records.
- Right to Confidentiality: You have the right to expect that all communications and records related to your care will be treated as confidential, except as required by law or to prevent serious harm.
- Right to Ask Questions: You have the right to ask questions about the assessment process, the science behind the protocol, the qualifications of providers, and the costs of services.
- Right to a Second Opinion: You have the right to seek a second opinion from another healthcare provider at any time.
- Right to Complain: You have the right to file a complaint about your care without fear of retaliation. Complaints can be directed to support@theresetregimen.com.
- Right to Data Control: You have the right to control how your de-identified data is used for research purposes. Consent for research use is separate from consent for clinical care and can be revoked at any time.
Your Responsibilities
- Provide accurate and complete information during the assessment process.
- Report any changes in your condition or symptoms promptly.
- Follow the care plan recommended by your clinician, or communicate your concerns about it.
- Seek emergency services (call 911) if you experience severe withdrawal, suicidal thoughts, or any medical emergency.
- Treat all staff and clinicians with courtesy and respect.
